Swing Beds: The Post Acute Care Lever

Rural Hospital Strategy Brief

Swing Beds: The Post-Acute Care Lever

Why swing bed programs matter to Critical Access Hospitals and the communities they serve

Swing beds let a CAH provide skilled post-acute care in its own inpatient beds, keeping patients close to home and adding volume to a largely fixed-cost unit.

The core questionIs your program sized to community need and run with a process that actively fills available beds?

Quality and cost create the value

Quality + Cost = Value

Most inpatient cost is fixed: unit staffing, medical direction, equipment, and supply par levels. Each added swing bed day brings mainly variable cost, such as supplies, drugs, and meals. Additional Medicare swing bed days can improve fixed-cost allocation and cost-based reimbursement; the benefit depends on patient mix, incremental costs, and proper Medicare cost reporting.

  • Skilled days · Medicare Part A
Payment101% of reasonable cost for covered skilled services
  • Skilled days · Medicare Advantage and commercial
PaymentContracted rate, which can add margin above incremental cost
  • Nursing facility days · Medicaid
PaymentVaries by state and may follow nursing facility payment methodologies. Proper cost reporting is needed to separate these days and costs from Medicare cost-based reimbursement.

When capacity is available, evaluate non-cost-based days against incremental costs, staffing requirements, and contribution margin rather than Medicare cost-based rates alone.

Program essentials

Six keys to a strong program

  1. Scope of careA multidisciplinary team, from medical staff and nursing to pharmacy, rehab, and the business office, defines which patients the CAH can serve.
  2. Active pursuitBuild relationships with PPS hospitals so you are the first call, and secure the return of patients you transfer out.
  3. Daily reviewCheck open beds and contact referring hospitals every day to find patients who need placement.
  4. CoverageVerify coverage, authorization, remaining benefit days, and applicable qualifying-stay requirements, including any waivers.
  5. Quality dataCAH swing beds are exempt from federal SNF MDS reporting but must maintain appropriate clinical assessments. Track discharge disposition, readmissions, ED visits, and functional improvement.
  6. Payer mixTrack days by payer and level of care, and judge non-cost-based days on contribution margin.
Admission process

A best-practice admission process

  1. IdentifyMatch open beds to patients awaiting placement at PPS hospitals, through the EHR or case managers.
  2. Clinical reviewConfirm that drugs, medical equipment, trained staff, and skilled rehab needs can be met.
  3. Business reviewVerify coverage, prior authorizations, and remaining benefit days.
  4. Provider reviewBring only screened patients to the provider, who determines medical necessity.
  5. PursueContact hospitals early, highlight your quality of care, and keep building ties with case managers and discharge planners.
Warning signs

When a program needs attention

  1. Open beds sit empty while nearby PPS hospitals hold patients awaiting placement.
  2. Referrals come from only one or two sources.
  3. Patients transferred out for acute care do not return for swing bed services.
  4. Non-Medicare days are declined because the rate is below the cost-based rate.
  5. No one tracks readmissions or functional outcomes.

Policy watch. A December 2024 HHS OIG report estimated potential Medicare savings of up to $7.7 billion over six years under alternative CAH swing bed payment rates. CMS did not concur with the recommendation.

Before you grow the program

Three checks before you expand

  • Define your scope of careAgree across departments on which patients you can serve well, and where you need resources first.
  • Assign an owner for pursuitName the person who reviews pending discharges daily and builds referral relationships.
  • Track and share outcomesMeasure readmissions and functional gains, and share results with referring hospitals and payers.
