Choosing a Primary Care Designation

Rural Primary Care Strategy Brief

Choosing a Primary Care Designation

A decision framework for FQHCs, provider-based departments, RHCs, and freestanding clinics

Each designation pays differently for the same visit, and the national RHC payment limit rises from $165 in 2026 to $178 in 2027 and $190 in 2028.

The core questionWhich designation produces the strongest net position for the whole organization, once Medicare, Medicaid, 340B, and cost report effects are counted together?

Start here

Fixed cost makes designation a revenue decision

Net benefit = Clinic payment + Hospital-level impact

Most clinic cost is fixed. Cost-based reimbursement primarily supports cost recovery, making total organizational margin, 340B opportunities, and cost allocation effects critical to evaluating designation strategies.

CAH cost-based facility paymentNoYesNo†No
Method II professional billing (when eligible)NoConditionalNoNo
Section 330 grantsConditionalNoNoNo

Conditional means it depends on eligibility. A PBD is a hospital provider-based department, not a separate Medicare designation.

*340B eligibility depends on the covered entity, clinic relationship, cost report treatment, and applicable registration requirements.

**All RHCs are now subject to a UPL.

†A PB-RHC is paid its all-inclusive rate, not a separate CAH facility payment.

Practice designations

Four designations, four rule sets

  • Federally Qualified Health CenterRequires: public or nonprofit status, service to an MUA or MUP, the required services, and HRSA governance and sliding-fee rules. Pays: FQHC PPS and 340B. Section 330 grants go to grantees, not Look-Alikes, and FTCA coverage requires deeming.
  • Provider-based departmentRequires: the 42 CFR 413.65 tests for ownership, control, and clinical and financial integration. Off-campus CAH sites must meet CAH location rules. Pays: CAH cost-based facility reimbursement, with eligible outpatient professional services at 115% of the Physician Fee Schedule under Method II, when applicable.
  • Rural Health ClinicRequires: an eligible rural location meeting shortage-area criteria, and NP, PA, or CNM coverage during at least 50% of operating hours. Pays: the lesser of the all-inclusive rate or the national limit. Qualifying grandfathered PB-RHCs keep a clinic-specific limit: their 2020 rate plus MEI, or the national limit if greater.
  • Freestanding clinicRequires: no location, APP staffing, or CAH distance test. Pays: the Physician Fee Schedule, with no 340B. Its cost report effect depends on how costs are allocated.
Red flags

When a conversion does not pay

  1. A PBD’s current rate still exceeds the RHC limit.
  2. The gain depends on 340B, and the 340B position is uncertain.
  3. The site fails rural, shortage-area, or CAH location tests.
  4. Grandfathered RHC status would be forfeited.
  5. Medicaid and cost report effects have not been modeled.

A note on integration. Medicare care management services add revenue under any designation, and PCMH status positions the clinic for value-based payment.

Strategic opportunities

Four opportunities, illustrated

  • +$129KIndependent practiceDesignationAn independent clinic gained $129K as an RHC. A hospital PBD converting to a new RHC would have lost $253K.
  • +$1.34MHealth systemRealignmentFreestanding clinics moved under a CAH as PB-RHCs. The gain includes $2.6M of 340B revenue.
  • +$37KCombined entitySpecialty integrationA behavioral health provider inside the RHC outperformed a separate practice.
  • +$27KAcquired practiceAcquisitionA modest gain that depends on 340B. The larger value is alignment and attributed lives.

Before changing a designation

Three checks before you convert

  • Model the whole organizationCompare every eligible designation side by side, including Medicare, Medicaid, 340B, and cost report effects, not just the clinic’s rate.
  • Confirm eligibility firstVerify location, shortage-area, and CAH distance tests, the 340B relationship, and any grandfathered status before building the financial model.
  • Retest as the limit risesThe 2026 limit of $165 now exceeds the illustrative $137 PBD average that once favored staying put. Revisit the decision each year.
